- Before discharge from hospital, the patient connects to their outpatient team. This enables a smooth transition to outpatient rehabilitation and can reduce the amount of time spent in hospital.
- The new outpatient team can view reports previously written by the inpatient team and quickly formulate a new rehabilitation program for the patient or edit the existing one.
- Therapy outcome measures can be monitored by the therapists and recorded within the platform.
- Therapists of all six disciplines have their own personal resource library which utilizes secure cloud storage. These resources can be assigned to individual patients to assist
them and their family members with at home rehabilitation.
- The centralization of the rehabilitation programme and associated media ensures that the patient can continue their therapy safely at home.
