A Catalan technology center with experience in information technologies applied to healthcare sector to take periodic clinical parameters remotely, easily and safely. Clinical users are able to manage gathered information to assist their patients and it is extremely easy to configure to create customized monitoring services for every healthcare purpose. Healthcare institutions, municipalities and health software providers are sought for technical cooperation agreements to perform pilot studies.


From 2012, the over-60 population will increase by about 2 million people a year and by 2020, around 25% of the EU population will be over 65. To improve their assistance and care of elder people and chronic patients it is urgent to promote home health care through ICT solutions.

The eHealth division of a Catalan technology center oriented to personalized medicine and integrated care (mHealth, teleassistance, etc.), has developed a technology to remotely monitor health status of chronic patients to trigger preventive, predictive and self-management interventions.
The target are patients living at home suffering chronic diseases such as cardiac failure, diabetes or COPD (Chronic Obstructive Pulmonary Disease).

The technology comprises a portable hub based on a tablet and the app which runs on kiosk mode to minimize technical problems and increase the reliability, and certified devices for taking clinical parameters (blood pressure, oxygen saturation, weight or glucose) and other devices like activity trackers or thermometer.
The application has different functionalities like personalized questionnaires to enhance medical review, notifications to promote patient adherence, videoconference tool to enable virtual medical visits, to take photos for clinical feedback and agenda.

The clinician can manage all the data through a cloud based web interface integrated with third party information system. The functionalities of the platform are:
- Personalized reports that can be downloaded or sent to third party systems
- Set alarms for every user and kind of data source (medical devices, questionnaires). Choose between different templates and enable e-mail notifications.
- Collected information is automatically displayed using colors and symbols to increase usability.
- Quick access to the kit status assigned to every patient.
- Validate new alerts and maintain historical data of all patient information.
- Audit trail available.
- Allow users with different configurations depending on the role and patients assigned.

The technology center is looking for healthcare institutions and software providers specializing in health for technical cooperation agreement to implement the technology in real environments through pilot studies.
